For Dental Providers

At Crown Discount Dental our goal is to connect trusted, reputable dentists with people in the community.

There is no charge to you for participating in the Crown Dental Plan.   All you will need to do is provide our members with a free Comprehensive Oral Examination (D0150) and Cleaning and Polishing (D1110), as per our current Fee Schedule sorted by diagnostic code.  Click here to view the schedule sorted by category. General Practitioners are asked to adhere to our Fee Schedule whenever the Procedure Code is indicated.  If the procedure code is not indicated, a 25% discount should be offered. Specialists are not governed by the Fee Schedule, but are expected to give a 25% discount to our members.

Our fee schedule is designed to help bring more cash flow into your office then traditional dental insurance, while still offering an affordable option for patients.

We calculate our discounts based on billing and survey information from thousands of US dental practices analyzed along side the pricing schedules from leading dental insurance providers. We have incorporated geographic data into our pricing methodology to account for regional cost of living variations.

Billing & Payments

Our members will pay you directly at the time of service.

How to Apply

Simply print out and complete our membership application and participation agreement and return them to us along with photocopies of your state license(s), DEA license and the face page of your malpractice policy.
